The process of mixing pasta with cheese and butter goes back to 15th century Italy. Martino da Como, a chef from Northern Italy, detailed a recipe for “Roman macaroni” or “maccaroni romaneschi” in his cookbook - Libro de Arte Coquinaria
Martino's recipe mentioned making pasta from scratch, kneading white flour shaping it into thin strips and boiling it in fatty broth or salted water with butter.
This simple yet rich and luscious preparation set the base for what eventually evolved into the modern Fettuccine Alfredo.
The modern Alfredo that we know today originated in early 20th century Rome by Alfredo Di Lelio.
After his wife gave birth, she needed a meal that nourishes her and Alfredo used only three ingredients like fettuccine, young Parmesan cheese, and butter.
This became popular internationally in the 1920s, as American actors Douglas Fairbanks and Mary Pickford paid a visit to Alfredo's restaurant.
They were so impressed that they gifted him golden utensils inscribed with “To Alfredo, the King of the noodles.”
What further garnered attention is Alfredo's tableside preparation of the dish.
Today there are two Roman restaurants, Alfredo alla Scrofa and Il Vero Alfredo, claiming to be the home of fettuccine Alfredo and compete with titles like “the king of fettuccine” and “the emperor of fettuccine”
In the U.S., fettuccine Alfredo became popular, often made richer with cream. Chains like Olive Garden helped it become a dining favorite, and jarred Alfredo sauces are now widely available. While some love its indulgence, others find it overly rich.
